By requesting a Health Assessment or Educational Consultation with Odin, I acknowledge and agree to the following terms:
1. Educational Purpose Only
I understand that the Health Assessment and Educational Consultation are provided for educational and informational purposes only. Odin does not provide medical advice, diagnosis, treatment, or medical supervision.
2. No Medical Provider Relationship
I acknowledge that Odin is not a medical provider. Submitting a request, completing a form, or participating in a consultation does not create a patient–provider relationship with Odin or any affiliated organization.
3. Investigational Nature of Regenerative Science
I understand that any discussion of stem cells, exosomes, or regenerative science is educational only and that these procedures are investigational and not approved by the U.S. Food and Drug Administration (FDA) for the treatment or prevention of any disease.
4. No Guarantees or Outcomes
I acknowledge that Odin makes no claims or guarantees regarding safety, effectiveness, outcomes, or results. Individual experiences may vary, and no results are promised or implied.
5. Personal Responsibility for Medical Decisions
I understand that I am responsible for consulting with a licensed healthcare provider regarding any personal medical decisions, conditions, symptoms, or treatments. Odin does not replace professional medical guidance.
6. Information I May Submit
I agree to provide only non-medical, non-protected information, such as my contact details and general areas of educational interest.
I understand that I should not submit medical information, including diagnoses, symptoms, medications, lab results, or treatment history.
7. Privacy and PHI Acknowledgment
I understand that Odin does not collect or store Protected Health Information (PHI) through forms, emails, social media, or chat tools.
If I accidentally submit medical or health-related information, I acknowledge that it may be deleted for privacy and compliance purposes.
8. Communication Consent
By submitting my information, I agree to receive educational communications from Odin related to regenerative wellness, research updates, and upcoming educational events. I understand that I may opt out at any time.
9. Independent Providers
I understand that any eligibility determination, medical evaluation, or procedure decision is made solely by independent, licensed medical providers, not by Odin.
10. Acceptance of Terms
By submitting a Health Assessment request or scheduling an Educational Consultation, I confirm that I have read, understood, and agreed to these Terms of Use.
